Parental Consent for Medication in School

If your child needs to take medication during the school day, please complete this form and return it to the school office – email or hard copy. Staff are unable to administer medication without written consent.

Please note: only medications that are essential during the school day should be sent into school. Where possible, medicines should be administered at home — for example, before and after school — to minimise disruption to your child’s learning and ensure safe storage and handling.

Allergy Information

Please complete this form if your child has any allergies that we should be aware of. This helps us to keep your child safe and provide appropriate care during the school day.
